Registration

The PARENT/GUARDIAN (hereinafter referred to as GUARDIAN) of the PLAYER/CHILD (hereinafter referred to as CHILD) a minor that I am registering. By completing registration I agree to the following release: WHEREAS, the GUARDIAN is desirous of having their said CHILD participate in the baseball program sponsored and conducted by the BOWLING GREEN PEE WEE LEAGUE ASSOCIATION, an OHIO not-for-profit corporation, (hereinafter referred to as the ASSOCIATION). NOW, THEREFORE, for and in consideration of the said CHILD being permitted to participate in the ASSOCIATION’S baseball program, and in consideration of the training and coaching said ASSOCIATION will give to said CHILD, and for other good and valuable considerations, the said GUARDIAN do hereby waive and release any claim or cause of action that they might have in the future against the said ASSOCIATION, its OFFICERS, COMMISSIONERS, COACHES, MEMBERS or its DIRECTORS by reason of their said CHILD being permitted to participate in the Baseball Program of the ASSOCIATION, including, but not limited to, any claim or cause of action for medical expenses, support, or wrongful death. The said GUARDIAN further agree to indemnify and save harmless the said ASSOCIATION, its OFFICERS, COMMISSIONERS, COACHES, MEMBERS or its DIRECTORS from any claims, suits, and causes of action that may be brought by, or on behalf of said CHILD against said ASSOCIATION, its OFFICERS, COMMISSIONERS, COACHES, MEMBERS or its DIRECTORS by reason of said CHILD participating in the ASSOCIATIONS Baseball Program.

Any intent to fraud or falsify any of the above information, will cause forfeiture of the Registration Fee and immediate expulsion from the league.
